Where does the phrase “fifty-fifty” come from and What does fifty fifty mean?

Fifty-fifty means: In equal amounts.

It is an Americanism which, since early in the twentieth century, has had the contracted meaning of 50 per cent for you (or one person) and 50 per cent for me (or another person).
